The Social Workers are available to assist both the residents and their families. The decision to place a loved in extended care is often a big step. The facility social workers are available to answer any questions or concerns regarding the facility. Assistance will be given with the admission process and to provide support to the individual and their family in adjusting to the facility. Providing for the residents’ psychosocial needs, the social workers will assist in information, referrals, and discharge planning.
Transportation
The facility has handicap equipped vans that are available to transport residents for medically related purposes. Family is required to accompany residents to their appointments if the facility van is being used. The facility does not provide staff to ride along with a resident except for the driver. If the van is to be used for transportation, all arrangements for the transport must be made by the Medical Records Coordinator in order to minimize scheduling conflicts with the transportation fleet.
